* To qualify for the residents / PhD students fee, please enclose an identification assigned by your head of training.

Included in the delegate / student registration fee:

• Coffee and tea during session breaks from Saturday to Tuesday
• Lunch boxes on Sunday and Monday

Payment

Registration fees, to be paid in EURO (€), should be paid according to the procedure described on the hard copy registration form included in this announcement. The same procedure will be described when registering on-line at the congress web site.

Confirmation of registration

When your completed registration form plus payment, or proof of payment, are received by the Congress Secretariat, you will receive written confirmation of your registration by mail. Registrations received by the Congress Secretariat after 15 August 2004 will not be confirmed in writing. This does not mean that you are not registered. Consequently, please bring the copy of the registration form, that you keep for your own files, as well as the proof of payment of your registration fee with you when you travel to Amsterdam.

Cancellations and refunds

The Congress Secretariat must be notified in writing of cancellations of registration:

• Before 1 July 75% of the registration fee will be refunded
• After 1 July: No refunds will be considered

Refunds are made after the congress.
